Which of the following most accurately describes the problem?

• Gwent-based JM Vokes has been warned that any further vehicle prohibition notices over the next two years could lead to the loss of its Operator's Licence.

The warning came at a Cardiff public inquiry, when South Wales Licensing Authority John Mervyn Pugh renewed the four-vehicle licence held by the company, of Upper Cwmbran, for a two-year period.

For the company, Terry Vaux said that assurances that Vokes would put its maintenance system in order, given at a previous hearing, had been complied with. Steps had been taken to improve the system substantially and three of the four vehicles had been put through the Mo'T test.

Taking no disciplinary action, Mervyn Pugh said that if everything were in order in two years' time the company could go back to a five-year licence. However, if it collected another prohibition, it would be back before him and its licence would then be in jeopardy.
