Which of the following most accurately describes the problem?

AWARNING against loading heavy concrete mixers on to lorries by using planks was given by Dr. J. Murray Robertson, West Surrey coroner, at an inquest at Windlesham on a man who sustained fatal injuries when a plank slipped and a mixer fell on him.

"There have been a number of deaths in Surrey, in similar circumstances, in the past few years," said the coroner. The jury recommended that in future safety measures should be taken.
