Which of the following most accurately describes the problem?

• The Vehicle Inspectorate is investigating an accident which left six dead. A Foden tipper hit a BT van in Sowerby Bridge, West Yorks and forced it into a Post Office. The victims included both drivers.

The Fewston Transport Foden 4000 Series was driven by agency driver Derek Waterworth, 63. He was a former policeman who held a Class 1 licence and had been driving for the company for some time, says director Tony Eyers.

Waterworth left Grassington early Monday morning with a load of stone and was going to Sowerby Bridge on a motorway maintenance contract for aggregates company Tilcon.

The truck hit the BT

van, which was driven by 42year-old Peter Stott, as it went over a mini roundabout, ploughed into some pedestrians and then into a Post Office. Two nearby houses were also damaged.
