Which of the following most accurately describes the problem?

• At the inquest at Lutterworth, Leics, on three people in a car and a van who were killed in a collision after a lorry tyre burst on M1 near Enderby, a witness from the Forensic Science Laboratory, Nottingham, told the coroner that the lorry's two front tyres were in poor condition. The casing of one tyre broke up, causing the tyre to burst. The tread broke away from the central casing and there was a split 2ft 4in. long in the tyre and it was found that the tread had been recut which, he said, was "an extremely dangerous practice".

According to the evidence, the lorry ran over the central reservation and its load of sulphur caught fire. This caused a collision between the car and van and the car also caught fire. The lorry driver did not give evidence and the coroner recorded verdicts of accidental death on all three deceased.
