Which of the following most accurately describes the problem?

by Karen Miles • Cumbrian police and Tesco are this week investigating an accident at one of the supermarket's stores in which a truck crushed a man to death.

Graeme Denwood, 31, died when a Tesco-operated Scania crushed him as its driver reversed into a loading bay at Tesco's Whitehaven shop in Cumbria.

It appears that Denwood, who was a Tesco staff member, was guiding in the vehicle and became lodged between it and a post in the loading bay.

The driver, who was treated for shock, is based at Tesco's Middleton depot in Manchester and has 30 years' experience. L A 72-year-old woman pedestrian was killed in Buxton, Derbys, when a lorry load of flour mix and train brake linings fell on her, leaving her injured beyond recognition.

The 38-tonne vehicle and trailer, operated by FR Somerset of Chapel-en-leFrith, lost control and mounted the pavement on the B5059 near the A6 junction, displacing a 10tonne load.

Margaret Lowe died later in hospital.

Company owner Robert Somerset says: "There's no reason why the accident should have happened, although conditions were icy. The driver was going slow but the vehicle lost control on a town road."
