Which of the following most accurately describes the problem?

• A Welsh driver was injured and his common-law wife killed after his lorry left the road, struck a railway bridge and overturned.

Gregory Kolton, a driver for Wrexham-based McCarthys Transport, was travelling along the A49 at 04:30hrs on 27 February when his empty !veto Ford hit a kerb and went out of control on a bend at The Grove, near Craven Arms, Shropshire.

The lorry then mounted the pavement before striking Stretford Bridge and overturning, completely blocking the road for over five hours.

Kolton's common-law wife Sharon O'Neil died from multiple injuries, and he and his two young children were treated at the Royal Shrewsbury Hospital for shock and minor injuries but released after treatment.

Leominster, Shropshire police say they are not sure what caused the lorry to leave the road, but added that it was a frosty morning and road conditions were very icy.
