Which of the following most accurately describes the problem?

West Midland LA John Mervyn Pugh adjourned consideration of disciplinary action against the licence held by Alan Cunliffe and Joseph Lively, of Hereford for a DOT vehicle examiner to be called after the partners said that a cracked wheel and loose wheelnuts were not due to a failure in their maintenance system. Cunliffe said the wheel had only been fitted to the vehicle 24 hours before a prohibition was imposed in a roadside check. The wheel had been checked twice, once by the tyre company when the tyre was fitted and once by the driver when the wheel was fitted. Wheels had to crack some time and it was unfortunate that it had happened after the checks, he said. Adjourning the hearing until August for the vehicle examiner to be called to give evidence, Mervyn Pugh said that the prohibition had been endorsed "S" as showing a significant failure in maintenance. Re directed that a full fleet inspection be carried out by the Vehicle Inspectorate.
