Which of the following most accurately describes the problem?

John Hayward of Troway, Sheffield

El South Yorkshire owner-driver John Hayward, of Troway, Sheffield, has had his licence renewed for six months, following an undertaking to have his vehicle inspected every week by a commercial garage.

In addition to Hayward's application to renew his international licence, in respect of one vehicle in possession with one to be acquired, Eastern LA John Mervyn Pugh was considering taking disciplinary action.

Evidence was given by a vehicle examiner that the vehicle had been given a delayed prohibition for an air leak at the relay valve when the handbrake was in the off position, and a fracture in a stud hole on one wheel Inspections, which were carried out by Hay ward to the best of his ability, were not satisfactory with little detail being recorded. The vehicle examiner was surprised to learn that the vehicle had passed its annual test the day before this visit.

The LA said that the vehicle, which carried roadstone and tarmac from quarries, was very old and doing very rough work. If he was not careful, Hayward was going to find himself without a licence because of his record and the nature of the work.

Renewing the licence until the end of next May, the LA said that if everything went well he might be persuaded to increase the inspection period to two weeks at that stage and renew the licence for two years.
