• West Midlands police has launched an investigation into last week's accident on the M5 near West Bromwich when two massive bridge girders broke free from a trailer and smashed through the central reservation, killing two car drivers.
The 37.5m girders, each weighing over 22 tons, were being moved from Tipton to Maidstone by Telford-based Wrekin Transport.
Ron Driver, the driver of the transporter, which was under police escort, escaped with shoulder and chest injuries when his Volvo F16 tractive unit tipped over.
The bridge girders were mounted on a four-axle 32-wheel remote bogie unit. "Initial indications appear to point to a mechanical failure," says a police spokeswoman.
The vehicle was travelling at 32 km/h (20mph) when it slewed sideways after rounding a bend between junctions one and two on the motorway.
The load then smashed into on-coming traffic.
