Which of the following most accurately describes the problem?

• Scottish bus operator William Buchanan, whose previous PSV Operator's Licence was revoked on maintenance grounds in August 1988 (CM 25-31 August 1988) was again in trouble over the maintenance of his vehicles when he appeared at disciplinary proceedings before Scottish Deputy Traffic Commissioner Ramsay Dalgety QC.

Buchanan, who trades as Buchanan Mini Coaches & Tartan Travel, of East Kilbride, was granted a fresh licence in January. Evidence was given that since last July, six immediate prohibitions had been placed on his three vehicles.

Dalgety said that he wanted Buchanan to institute a driver defect reporting system. Issuing Buchanan with an "absolutely final" warning, and renewing the licence for three vehicles for one year, Dalgety said he hoped that Buchanan was under no illusions about what would happen if there were future problems. He said he would not tolerate buses that were regularly going on the road in an unsafe condition.
