Which of the following most accurately describes the problem?

• A new motorway slip road linking the M40 with the M42 has been condemned as "an invitation to suicide" by a local MP — because the slip road joins the outside lane of the M42 motorway.

Selly Oak MP Anthony Beaumont-Dark says: "This road has got to be redesigned. I am not going to stand by and watch people get killed." And Road Haulage Association Midlands district manager Robert Ward says: "The slip road is an accident waiting to happen."

The slip road is especially hazardous for HGV drivers: being prohibited from the third lane they have to move over to an inside lane as soon as they join the motorway.

The Department of Transport will monitor the situation.
