Which of the following most accurately describes the problem?

• A driver who lost his load of dangerous chemicals in the centre of Middleton, North Manchester has been banned from holding an HGV licence for six months.

A Manchester public inquiry was told that this was the third instance of an insecure load involving Frederick Jordon of Norton Avenue, Morecambe. The Middleton incident also resulted in Jordan being prosecuted for driving without due care and attention.

Asked by North Western LA Martin Albu how he had managed to be convicted on three occasions of having insecure loads, Jordan said that his mind had not been on the job and he had not checked the loads as he should have done. Albu said that the insecure loads could have caused very serious problems.

The LA said he could not overlook what had occurred — Jordan had made the same mistake three times.
