Which of the following most accurately describes the problem?

• After pleading guilty to operating a goods vehicle which was not specified on an O licence, Colne Valley Urban District Council, Lanes, was fined £20 by magistrates at Huddersfield West Riding Court on Tuesday.

Mr Michael Patterson, prosecuting for the Licensing Authority, stated that the offence came to light on October 16 last year when the vehicle in question LCH 936K was seen carrying a load of grass and earth at Slaithwaite, Yorkshire. At that time, he said, the council held a licence specifying five vehicles with a margin of one to be acquired, but this did not include the lorry involved in the offence. Mr Patterson added that on October 26 the council successfully applied to the Yorkshire LA to remove one specified vehicle from the licence and adding two further vehicles including LCH 936K.

When cautioned, Mr K. Willis, a council employee responsible for transport, said: "This was an oversight due to a change of staff."
