Which of the following most accurately describes the problem?

Min this week's issue (page 32) AI Medina Transport won back its repute at an appeal before the Transport Tribuna. It ruled the TC had placed too much importance on its past failures and had not taken into account the "positive features" which showed the company was "both willing and able to address what had gone wrong".

III A Midlands scaffolding firm lost its repute for a minimum of six months because it ran without an 0-licence for nine months (CM 1 July).

• An operator narrowly held on to his repute, and his licence, despite having been transport manager of a County Durham company whose licence was revoked (CM 10 June).

• A South-Eastern operator lost its repute and a director and former director were banned indefinitely from holding or obtaining 0licences. The TO said the company's refusal to comply with undertakings relating to hours and tachographs, and its failure to co-operate with VOSA, "went to the heart of the 0-licensing system". It didn't help that the company had provided photocopies instead of originals when asked for tachograph charts (CM3 June).
