Which of the following most accurately describes the problem?

Iwonder if you could give me your opinion on this incident. A lorry and trailer go in for an inspection and to have whatever is wrong put right—brakes adjusted, greased up and so on. The the following day the lorry is stopped by the Ministry and found to have faulty brakes—the collects were left in the down position on two slack adjusters and they wound themselves off, leaving two cold brake drums.

Where does the fault lie? With the operator, the engineers or the driver for not finding the badly adjusted brakes on his daily check ?
