Which of the following most accurately describes the problem?

• An owner-operator whose truck jacknifed after he tried to drive under a bridge too low for his load has been jailed for causing death by reckless driving.

Peter Sagar, from Lancashire, trading as P Sagar Haulage of Tarleton, was carrying a container giving his artic a total height of 4.42m, but the railway bridge on the A635 Doncaster to Wakefield road was only 4.27m high, Leeds Crown Court heard. The container came off his vehicle, which jacknifed. A doctor driving in the other direction ran into the tractive unit and was killed.

Sagar was told by the judge at Leeds Crown Court that he had not taken the elementary precautions of checking the height of the container or setting the height meter, and was sentenced to six months jail and banned for two years.
