Which of the following most accurately describes the problem?

• At Helmsley last week verdicts of accidental death were recorded at the inquest on eight elderly coach passengers and two occupants of a car which was crushed when the coach careered into it following a coach crash a year ago.

Mr F. Beardsell, a vehicle examiner, said the rear wheel brakes of the coach had "a limited period of operation" while the brake linings of the front wheels had not been in contact with the brake drum "for some considerable time" due to the adjusters being broken. In addition parts of one tyre had no tread and six out of eight nuts on one of the wheels were slack.

Mr Geoffrey Newey, the North Riding Coroner, said the driver of the coach had been found not guilty of causing the deaths of two of the victims. He had not been charged with causing the deaths of the other ei ht.
