Which of the following most accurately describes the problem?

• A workman was crushed to death beneath a steel pipe when it fell from a pile being loaded on to a truck.

The incident happened at around Sam last Tuesday (15 April) at Findlay Fabrications on Aycliffe industrial estate in County Durham. David Applegarth was securing the steel pipes onto the back of the vehicle when one of them, weighing a quarter of a tonne, came loose and fell on him, throwing him 10ft to the ground. Another pipe then rolled down from the vehicle on top of him.

A spokesman from Durham Constabulary says: "The man suffered severe head irtjuries and was pronounced dead at the scene by a police surgeon."

The Health and Safety Executive has launched an investigation into the incident.
