• A transport company manager ordered a cover-up of maintenance records after a truck was involved in a crash on the M5 Avonmouth Bridge in which a mother and daughter died, a court heard.

Martin Howell, transport manager of TO Howell and Sons, based in Newport, South Wales, phoned David Dugmore, owner of the maintenance contractor which looked after his company's vehicles, and told him to "get the records straight", Bristol Crown Court was told.

Ann Hughes, 67, and her daughter Angela, 34, of Swansea, died when their Rover Metro was crushed in the middle of the five-vehicle pileup on the M5 on 1 September, 2000.

The court heard how the crash happened when an unidentified white car suddenly switched lanes. The artic belonging to TG Howell and Sons crashed into the back of the Metro, wedging it under a van and into the back of another truck. A fire broke out because of a fuel leak. Ian Glen QC told the court that police found 69 of Dugmore's 142 maintenance record sheets for the artic and 13 other of Howell vehicles had been falsified.

Howell denies two charges of breaching health and safety legislation and conspiracy to pervert the course of justice with David Dugmore.

Dugmore, of Abercarn, Gwent: pleaded guilty to that charge at an earlier hearing. TG Howell and Sons also denies two charges of breaching hearth and safety pegs. The hearing continues
