Which of the following most accurately describes the problem?

Few operators escape P09s completely. But if you receive one, ask yourself: how could it be prevented in future?

• Could the driver have prevented its issue or could the fault have been picked up only by someone more qualified on a more detailed inspection?

• Look at the last safety inspection sheet or defect report for that day to see if it was noted.

*Are the maintenance inspections at appropriate intervals or do they need to be done more frequently?

• Do staff need more training or should you consider disciplinary action? 0 Are defects being reported but not being rectified promptly enough?

• Is there a pattern to the nature of the prohibitions and, if so, are there additional measures that can be taken? For example, roller brake testing: a few pounds spent now could save a fortune in the future.
