Which of the following most accurately describes the problem?

• Scottish Licensing Authority Hugh McNamara has issued a warning to Ben McLuskey & Sons of Almondbank following the company's appearance at disciplinary proceedings in Perth because of problems over maintenance.

The company holds a licence for six vehicles and three trailers and the LA was told that six prohibitions, including one immediate prohibition, had been imposed on its vehicles and trailers.

During a maintenance investigation in March, inspection records for one vehicle could not be produced, and that vehicle attracted a delayed prohibition. Vehicles were inspected every four weeks or 6,500 kilometres, and trailers every eight weeks or 13,000 kilometres.

For the company, it was said that a file of inspection records had been lost. The immediate prohibition related to tyre defects on a trailer. One of the tyres concerned was of a particular type that was causing a lot of problems. Undertakings were given that in future drivers would check over their vehicles daily, that the fifth-wheel couplings would be inspected at least once a week and that the trailer inspection frequency would be reduced to every four weeks or 8,000 kilometres.
