Which of the following most accurately describes the problem?

• The danger of vehicle recovery work was highlighted again last week when a driver was killed on the M4 hard shoulder while helping a couple who had run out of fuel.

Peter Hughes, 41, of Sloughbased recovery operator Courts died at the scene when a driver of a 7.5-tanner drove into his recovery vehicle, shunting it into Thomas Sharkey's car which in turn hit the two men. Sharkey died in hospital.

Mrs Sharkey, who had remained in their car, was treated for severe shock. The van driver, Andrew Exton, 43, of Chippenham, Wiltshire, suffered a fractured leg.
