I/We, the undersigned, hereby request you to Licence to stand and ply for hire within the County Borough of Middlesbrough (subject to the Bye-Laws and Regulations made by the Town Council in that behalf) the, following Hackney Carriage (e) ,of which L am/we are sole (or one of the) Proprietor(s). And I/we state (a) that no person(s) other than me/us is/are interested in such carriage(s) (or) (b) that the names and addresses of all other persons interested as Proprietors or part Proprietors in the said Hackney Carriage (s) are set out below:—
NOTE.—The Proprietors of Hackney Carriages (which includes Motor Omnibuses) with their respective carriages, etc., and the horses or other animals intending to draw the same will be required to attend for inspection before the Public Service Miele Inspector, or before ' the Licensing Committee of the Town . Council, as and when required by the Council's Inspector.
