Which of the following most accurately describes the problem?

II The Driver Vehicle Licensing Agency has reported a second security breach in two weeks after 3,500 passports were lost. This follows the loss of a number of V5 vehicle registration forms, some of which were used to register stolen vehicles (CM5-11Sept).

The 3,500 passports were sent to the DVLA as proof of identity with licence applications. It is not known if they have been lost in the post or were intercepted.

The DVLA says that it takes the issue of missing documentation extremely seriously but has "no reason to believe that any crimin activity has been involved".

It adds that in the last financial year con pensation cases involving lost clocumen1 equated to 0.06% of the 5.4 million drivir licences issued.

However, to improve security passport can now be checked at the Post Office.

A spokesperson for Consignia says: "Le5. than 0,1% of the 21 billion items delivered eac year go missing. We are working hard 1 reduce this number."
