Which of the following most accurately describes the problem?

• An inquiry has been ordered into the state of vehicles run by Midland Red following an accident in which two people died when a driverless bus ran into a bus queue at Dudley, Worcs.

The West Midlands Traffic Commissioners have called the company to a hearing on November 30 in Birmingham.

Midland Red, operating 1650 vehicles, was told to take 17 buses off the road when DoE officials examined them following the accident in September.

The Commissioners have warned the company that some or all of their passenger service vehicle licences could be withdrawn.

Inquests are to be opened shortly on a seven-year-old girl and a 62-year-old woman who died when the bus careered down a hill and into the bus queue.
