Which of the following most accurately describes the problem?

• The vulnerability of children and short people near trucks has been highlighted for the second time in as many weeks after a young brother and sister were crushed under the wheels of a tipper. Five-year-old Candle Welsh and her six-year-old brother William died in Edinburgh when a tipper working for locally based Anderson Haulage crushed them as it moved forward. The vehicle's dryer was admitted to hospital for shock.

Lothian and Borders police is preparing a fatal accident report for the Procurator Fiscal Service, the Scottish prosecuting body, and Anderson Haulage is meanwhile declining to comment publicly.

The deaths occurred just days after a driver delivering to a store in London was found blameless after crushing a 4ft-tall woman, and a 38-year-old woman suffered multiple injuries after being crushed against the bulkhead of an Irish ferry as a lorry was reversing.
