Which of the following most accurately describes the problem?

A MANAGER, "whose efforts were not directed in the company's interest," did not inform his managing director that two vehicles in his fleet had been issued with GV9s. The South Eastern LA heard at Reading that two vehicles based at United Carriers' Alton transport depot were inspected by a vehicle examiner and found to be making excessive smoke. In addition, the exhaust system on one vehicle was insecure. The LA Mr G. C. Mercer deleted three vehicles from the operator's licence for one month.

The managing director of United Carriers, Mr Jack Stanforth, said that since he had heard about the fall in maintenance standards at Alton he had had many "sleepless nights". He had only heard about the GV9s when the depot manager had been replaced, six months after the prohibitions had been issued. Mr Stanforth cited the fact that United Carriers have recently obtained renewals of operator's licences in four other areas as evidence of the real maintenance standard of the company.

The new manager of the Alton depot, Mr J. Sabey said that the backlog of work had been attended to and new system of spot check: had been adopted to mak( certain that maintenance standards were adhered to.
