Which of the following most accurately describes the problem?

• Maintenance problems have resulted in Hordean-based Castle Coaches' vehicle licence being chopped from 14 to four by South Eastern Traffic Commissioner Brigadier Michael Turner.

One prohibited vehicle was twice refused clearance because of neglect, said a traffic examiner, and a coach that failed its annual test was represented with two of the original major defects still present. The company blamed staffing problems and said it intended to extend the workshop and improve its equipment. Cutting the licence, Turner said the company directors had failed to heed a warning given at disciplinary proceedings in June 1987 following previous maintenance problems and tachograph-related convictions. They seemed content to shift the blame onto their fitters, said Turner.

He had been "within a whisker" of revoking the licence altogether, but the company's plans had persuaded him to allow it to continue with a much reduced operation. This was its last chance to stay in the PSV business, he warned.
