Which of the following most accurately describes the problem?

Office has lammed the Department of Transport, Scottish Development Department and the Welsh Office for poor supervision and workmanship of road and bridge building. In an analysis of 210 roads and bridges which had to be repaired earlier than planned, the NAO says that 2190 million had to be spent because of weaknesses in design and specification. Another 296 million was spent on strengthening DTp bridges built in the 1960s and 1970s. Some of the early maintenance was due to unexpectedly heavy lorry flows and loadings, says the report.
