Which of the following most accurately describes the problem?

• A Derbyshire haulier whose defective truck killed a local woman is to face a public inquiry in Bakewell today (12 January).

The haulier, Bakewell-based HG Pheasey, was found guilty in September last year of operating a cement-laden tanker with defective brakes and defective steering with other offences and fined .0,300.

Derby Crown Court had previously sentenced the driver of the tanker. Robert Marsden, to 200 hours community service for causing death by reckless driving and he was disqualified for four years.

Susan Williams, a university lecturer, was crushed to death by the tanker when it toppled on to her car on the A619 at Heathey Lee near Baslow on 22 May 1992.

An immediate prohibition was placed on another of the company's vehicles in March 1993. The public inquiry will consider disciplinary action against the company during the inquiry.

The haulier has also applied to increase its fleet by two vehicles and two trailers from its current authorisation of three vehicles and two trailers.
