Which of the following most accurately describes the problem?

AN ESSEX truck body firm has been fined £7,500 after an employee suffered life-changing injuries when he fell from a stepladder while spray-painting a lorry.

In a Health and Safety Executive (HSE) prosecution, Chelmsford Magistrates’ Court was told how the 51-year-old worker from Latchingdon, who has asked not to be named, was painting the LGV at the Chelmer Truck Bodies site in Boreham near Chelmsford in January when one of the feet on the stepladder slipped down a grille in the concrete floor.

The man fell approximately two metres, shattered his left shoulder and collar bone, broke several ribs and received a deep cut to his head.

He is unable to return to work and suffers considerable pain due to his shoulder injury. The court was told the company should have provided safe and secure working platforms rather than lightweight stepladders to carry out the work, as staff were working at height for several hours at a time.
