Which of the following most accurately describes the problem?

A WELSH OPERATOR which failed to turn up at a Cardiff disciplinary inquiry has had its licence revoked. Vixen Flag of Newport, Gwent, had been called on to explain its poor maintenance systems and a string of prohibitions.

The company, which held a restricted licence for four vehicles and one trailer, had been called before Welsh Deputy Traffic Commissioner Alan Jenkins. Vehicle examiner Robert Goff said there were only parking facilities at the company's base; it seemed that maintenance was undertaken by a company in Bridgend, some 40 miles from Newport. There were no written inspection records, no maintenance

flowchart and few driver defect reports. In one case it appeared that no action had been taken, despite the same vehicle defect being reported on eight occasions during March and April last year.

A number of immediate prohibitions had been issued for oil leaks, a cracked windscreen, inoperable indicators, cut tyres and loose wheelnuts. Several variation notices had been issued when vehicles were presented for clearance.

Revoking the licence, the DTC said a traffic examiner had reported problems in trying to contact the company and claimed its vehicles had been used unlawfully on hire-and-reward work.
