Which of the following most accurately describes the problem?

• Welsh bus company Shamrock Private Hire (Newport) has been fined £1,500 after admitting failing to maintain the brakes on a single-decker which was involved in a bus crash in 1988, leaving nine people injured.

Pontypridd magistrates heard that the bus had crashed into a stationary minibus on a steep hill, pushing it over 30m into a house. The bus carried on and hit a truck before demolishing a library.

Owner Clayton Jones said the vehicle had been for its annual test a month before the accident and regular checks had not shown any brake defects.

He said moral responsibility rested with the driver at the time, who was jailed for reckless driving.
