Which of the following most accurately describes the problem?

• Greylands Waste has had its 20-vehicle licence revoked and the company and directors Michael O'Driscoll and Martin McCarthy have been disqualified from holding or obtaining a licence for two years. The company, of North Woolwich Road, Landon, appeared before South Eastern & Metropolitan LA Brigadier Michael Turner at an Eastbourne disciplinary inquiry following the issue of 19 prohibition notices since the licence was granted in February 1993. Last October the LA cut the duration of the licence to expire at the end of November. Since then one delayed and six immediate prohibitions have been issued. Vehicle examiner William John said he had issued one delayed and two immediate prohibitions during a maintenance investigation in February for loose wheelnuts, a broken spring, and braking and steering faults. For the company, it was said maintenance problems had arisen because of an enforced move to a new operating centre. Brigadier Turner said the company had a clear warning last October and had Failed to take the remedial action The vehicles were not being kept in a fit and serviceable condition and the directors had to take a major share of the blame.
