Which of the following most accurately describes the problem?

AN OPERATOR who was jailed for falsifying maintenance records after a fatal accident in Southampton, has lost his 0-licence and been banned indefinitely from holding a licence in any Traffic Area.

Paul Duckmanton, trading as Rotherham-based CarTrans, had been sentenced to eight months on t WO counts of attempting to pervert the course of justice. His workshop manager, Terence Gleadall, was jailed for four months on similar charges. Michael Roberts, who drove the vehicle involved in the accident, was sentenced to 12 months for manslaughter (CM 14 October 2004).

Joyce McVey was killed instantly when a CarTrans truck carrying seven Land Rover Discoveries failed to stop at traffic lights in Southampton in September 2002.

Only two days before, the same vehicle, again driven by Roberts, was involved in an accident at exactly the same j unction. Roberts reported it to Duckmanton and complained about the brakes; another driver later looked at them but found nothing wrong.

Making the revocation and disqualification orders, TC Tom Macartney said Duckmanton had allowed the falsification of inspection records and was unfit to remain in the industry.

Failure to arrange an inspection of the vehicle by a qualified engineer was a grossly irresponsible act. Duckmanton had ignored pleas for additional resources; and had ignored the risk his vehicles represented to the public.
