Which of the following most accurately describes the problem?

• Police are appealing for witnesses to a fatal accident at the Dartford crossing in Kent which killed an 11-year old girl and left five others injured.

Michelle Shields and three friends had just crossed the Queen Elizabeth II Bridge into Kent last Friday afternoon (16 August) when a 7.5-tonne Ford Cargo towing a trailer overturned and landed on their ea' as they were pulling away from the toll-booth.

All four occupants were trapped for two hours. Another three vehicles were involved in the pile up.

The Ford Cargo driver, Kenneth Garden, who works for SCM Caterers in Northampton, escaped with minor injuries. Garden had been part of an SCM convoy taking trailers to an exhibition site. The Vehicle Inspectorate officers are examining the vehicles and police would like to hear from any truck drivers who were crossing the bridge at around 16:00hrs on Friday 16 August.
