The court had heard of another incident three months before Robinson's death when a I5-yearold girl was also hit by a reversing council lorry. She escaped with minor injuries.

Council chief executive Stella Manzie says it now uses reversing assistants and has introduced internal checks and briefings of refuse crews, which include practising reversing manoeuvres. Manzie also says drivers have been issued with pocket-sized handbooks focusing on health and safety measures and the council is piloting radar-based mechanisms to assist drivers in reversing.

We offer our continued sympathies and regrets to Amy Robinson's farni ly," she adds."This was a tragic accident and we have co-operated fully with the West Midlands Police and the Health and Safety Executive(HSE) to ensure there were no obstacles to a complete and open investigation." The HSE, which brought the prosecution, says the waste industry must ensure adequate precautions are taken during reversing manoeuvres.

USE inspector Paul Smith says: "There is a tragic history particularly of children and old people being run over by refuse collection vehicles. These incidents must act as a stimulus for the industry to review its procedures."
