Which of the following most accurately describes the problem?

by two vehicles has been turned down after West Midland Traffic Commissioner Nick Jones expressed concern that the company had many brake-related prohibitions.

However the TC decided against taking action against the existing licence held by Stoke-on-Trentbased AG Plant Hire.

'the new application sought to increase authorisation from four vehicles and two trailers to six vehicles and two trailers. Vehicle examiner Neil Brown said that during an arranged maintenance investigation in May he examined two vehicles and a trailer, issuing two immediate prohibitions, one delayed prohibition and a number of defect notices. One vehicle was given a variation notice when produced for prohibition clearance. He also found that a number of inspection records had been inadequately completed.

Prior to this, a vehicle had been given an immediate prohibition in April 2006 for brake and tyre defects. A variation notice was issued when the vehicle was subsequently presented for prohibition clearance.

Brown agreed with John Dyne, for the company, that there was no evidence of significant maintenance failures and that many of the defects were of a recent nature, -TheTC said he was concerned by the poor outcome of the maintenance investigation considering it had not been unannounced: he was also concerned that many of the prohibitions were brake-related.

Director Andrew Gimbert said he was aware the vehicle examiner was going to inspect the fleet and had told the transport manager and the company's fitter to make sure everything was right. He was very disappointed and had given the fitter a written warning. They had taken on an additional fitter since and Girn bert personally now checked things daily. The vehicles were now sent for roller brake tests every other safety inspection. A deputy transport manager had been appointed who would be sent on a CPC course.

Transport manager Michael Boon said he thought they had now addressed the problems.
