Which of the following most accurately describes the problem?

A BED manufacturer has been handed a formal warning after a maintenance investigation revealed defective systems and poorly maintained vehicles, including brake defects.

The investigation was sparked by the poor annual test history of Bradford-based Silent Sleep, which operates four vehicles. The company appeared at a Leeds disciplinary inquiry before North Eastern Traffic Commissioner Tom Macartney.

Vehicle examiner Gareth Dodd said he had examined three vehicles, issuing one immediate prohibition, one delayed prohibition and two defect notices for brake defects. One vehicle had last been given a safety inspection in July. There was no written driver defect reporting system and the wallchart was not up to date. The inspection records were only kept for 12 months and not the required 15.

Dodd prohibited one vehicle which had been inspected just four days before his visit by the van maintenance contractor. But he accepted the main tenance was now being contracted out to a reputable commercial garage.

Silent Sleep director Tariq Hussain said the company had terminated its previous maintenance contract after the vehicle examiner's visit. It had cost up to £10,000 to get the vehicles repaired. He agreed help from a transport consultant would be useful.

The company was given a formal warning after Hussain gave undertakings, including that driver training will be handled by an outside agency.
