Which of the following most accurately describes the problem?

TANKER DRIVERS who were at the Buncefield oil depot on 11 December seconds before the explosion should have been warned of a fuel leak by an alarm.

CM has been told that the inquiry into Europe's largest peacetime explosion will almost certainly conclude that the blast happened as a result of human error, but the drivers are not expected to be blamed.

The explosion was started when fuel vapour ignited, but the Health and Safety Executive (HSE) has not yet released its findings on why the fuel leak occurred.

Possibilities include a hole in a fuel tank, an over-lilled tank, or a tank with an open tap or valve.

However CM understands that if a tank was leaking an alarm should have activated.

The site is operated by oil company Total.

The HSE has produced a progress report on the Buncefield explosiomits next report is due out in May.
