Which of the following most accurately describes the problem?

• The Health & Safety Executive is investigating a series of accidents at Manchester Airport's oar parks: one resulted in serious injuries to the driver of an artic dumper.

Firemen had to cut the 17year-old driver from the dumper after its roll bar collided with an overhead gantry at Terminal 2's multi-storey car park.

The gantry collapsed, inflicting severe head injuries on John Flynn, who was on a landscaping contract near the terminal.

A spokeswoman said the same height restriction gantry had been hit by a minibus some weeks before the incident last month. In March the roof of an airport bus was sliced off by a gantry when the driver took a wrong turning. The spokeswoman was unable to confirm the same gantry was involved; or that a van became wedged under another gantry.
