Which of the following most accurately describes the problem?

A WASTE TRANSFER company has been fined £50,000 with £30,000 costs after one of its drivers died when he was buried in waste.

According to a Health and Safety Executive (HSE) investigation into White Reclamation, part of the White Group, the company had "a significant lack of control of vehicle movements" at its Eccles site.

Richard Buckley, an HGV driver for the firm, was emptying his vehicle on the site when he became buried in the waste.

Manchester Crown Court heard that at the time of the incident, there was no segregation of pedestrians or vehicles and no traffic management systems. The lack of control meant immediate action had to be taken to minimise the risks. White Reclamation pleaded guilty to breaching sections of the Health and Safety at Work Act.

A spokesman for the White Group says it would not comment until it had received a full transcript from the court.
