Which of the following most accurately describes the problem?

A VERDICT OF accidental death has been returned by a coroner's inquest into the death of an 84-year-old man who was run over by a reversing truck while walking in a London park.

St Pancras Coroner's Court heard that Ben Glass was strolling in Golders Hill Park last March when he was knocked over by a 17.5-tonner operated by Welch's Transport.

Driver Anthony Brewer had been delivering supplies to the nursery in the park but, hu iuse of building work, he was unable to exit through the top entrance. There was nowhere to turn around in the yard so he reversed along the pedestrian path, but without the aid of a banksman.

The road was narrow, and trees and rubble may have obstructed his view. The truck's reversing beeper was not working; Brewer had reported this to his employer a month before the incident.

Glass became trapped under the rear of the vehicle and was dragged along the ground. He sustained multiple injuries and died in hospital the following day.

Brewer had previously pleaded guilty to careless driving and received a £180 fine with seven penalty points.

Coroner Andrew Reid is to write to the Corporation of London, which owns the park, and to Welch's Transport about reviewing their working practices.

Welch's declined to comment.
