Which of the following most accurately describes the problem?

• A lorry driver blamed For a motorway crash in which two people died has been fined "a totally inadequate" 2,500.

The drivers of a van and a car were killed in the Four-vehi cle pile-up when Dean Milbourne lost control of his artic as he approached stationary traffic on the A1(M) in County Durham.

Teeside Crown Court heard that Milbourne's vehicle struck a van driven by electrical contractor John Holt, which in turn hit a car with 54-year-old Patrick Hunt at the wheel. Milbourne, 33, of Clacton, Essex was convicted of two careless-driving offences. He was fined £500 and banned from driving for a year.

"This will be seen by those who have lost people they loved as a totally inadequate penalty," Judge David Bryant told him. "Essentially, I am not passing sentence for killing two people; am passing sentence for a piece of extremely bad driving."

Jamie Hill, defending, said: "It wouldn't have taken more than a few seconds inattention to lead to this disaster."
