Which of the following most accurately describes the problem?

A POOR annual test pass rate and extended periods between safety inspections has resulted in the licence held by Oldhambased Barnes Logistics being cut by six vehicles and two trailers.

The company had been called before North Western Deputy Traffic Commissioner Mark Hinchliffe. It held a licence in the North West for 30 vehicles and 30 trailers, with 16 vehicles and 20 trailers in possession, and was seeking a one-vehicle licence in the North Eastern Area.

Vehicle examiner Steven Ainscow said some trailers were not being inspected at the stated interval of 12 weeks, with the longest gap being 16 weeks.

Eight vehicles and four trailers were examined in December. One immediate and two delayed prohibitions were issued, all of which were removed during the visit.

The initial pass rate at annual test was 42.86%. Maintenance was contracted out and the company was unaware of the poor initial pass rate. Over the past five years, five immediate and two delayed prohibitions had been issued to its vehicles and trailers.

When he visited the company in April it had brought in an MOT tracking system, failure report sheets, a computerised spreadsheet to monitor safety inspection dates, and the drivers' defect reporting system had been updated. It had also held meetings with its maintenance providers outlining the level of service it required.

Director and transport manager Christopher Barnes said he had been trying to run the business and take on the responsibility of transport manager. It was too much and a new transport manager, Matthew Bourne, had been appointed.

Barnes gave a series of undertakings to the DTC in relation to the company's maintenance arrangements.

The DTC cut the authorisation on the North Western licence to 24 vehicles and 28 trailers and granted the North Eastern licence.
