Which of the following most accurately describes the problem?

A FIRM has been allowed to operate just one vehicle out of four after a disciplinary inquiry to explain three unsatisfactory maintenance investigations, including a dangerous case of missing wheel nuts.

Richard and Leslie Morgan, trading as Morgan Bros, of Montgomery, were called before the Welsh Deputy Traffic Commissioner Roger Seymour at an Oswestry disciplinary inquiry.

The DTC was told that there had been unsatisfactory maintenance investigations in May 2002, September 2004 and December 2004. In addition one delayed and three immediate prohibitions had been issued, all of which were marked, together with a variation notice. Very few defects were listed on the inspection records, which were not a true reflection of the condition of the vehi cles.The initial failure rate at annual test was 60%.

After Richard Morgan had said that the vehicle were now being checked every two weeks, the DT( pointed out that if the six weekly safety inspection were carried out properly the defects would not hay arisen. Commenting on a prohibition for six out of I, wheel nuts missing with wheel loss imminent, the DT( said it was death and destruction waiting to happen.

Morgan's son-in-law, Roger Page, a self-employei mechanic,conceded that the quality of his inspection had been less than satisfactory in the past. Morgan un dertook to reduce the period between inspections t. four weeks and to install lighting in the inspection pi and paint the walls white to improve visibility.

Cutting the licence from four vehicles to one, th DTC said that the vehicles had been a danger and nu action had been taken to eliminate that danger.
