Which of the following most accurately describes the problem?

A Shell tanker driver caused chaos at the weekend when he accidentally filled up a garage's unleaded petrol pumps with diesel.

At least nine vehicles ground to a halt and were forced to call in the emergency services after filling up at the Shell service station off the M2 in Wigmare, Kent.

It was discovered that the driver had poured 10,000 litres of diesel into the garage's unleaded tank.

A spokesman for Shell says: "We try to do all we can to avoid events like these happening through training. It was a genuine mistake and we are very sorry." Motorists were compensated with hire cars and full tanks of petrol.

"The fuel has now been flushed out [of the garage's tank] and safeguards are in place to ensure that no traces of diesel are left in the tank. The diesel will be disposed of off-site."
