Which of the following most accurately describes the problem?

Andreas Klassen, 51, was said to be doing 54mph when he smashed into stationary traffic near the M6 junction with the M6 Toll road on 10 September last year. Tachograph records showed that Klassen had been driving for nearly 23 hours with only a 5hr 41min break.

At Staffordshire Crown Court Klassen pleaded guilty to four counts of causing death by dangerous driving; he was also banned for five years.

Judge Mark Eades remarked that the only reason for the fatal crash was that Klassen had fallen asleep at the wheel.
