Which of the following most accurately describes the problem?

A worker was killed last week when an Unmanned tipper truck and a refuse truck collided at a rubbish tip in Lanarkshire. Another man suffered back injuries when he was struck by one of the vehicles which recoiled after the collision at a landfill site run by Caird Environmental, at Riggend near Airdrie.

Both men were taken to nearby Monklands Hospital. The incident is being investigated by the Health and Safety Executive.

The worker, Jamie Skivington, had worked for Caird for nine years.
