Which of the following most accurately describes the problem?

• A driver who lost control of his truck on a corner ploughing into a car and killing its three occupants has been jailed for three years. Peter English, of Bradley Stoke near Bristol, admitted careless driving when he appeared at Gloucester Crown Court before Christmas. Police found his crumpled tacho chart in a nearby hedge following the accident near Tetbury, last March. It showed he had been travelling at 67mph and, despite braking heavily, was still going 47mph at the time of the collision.
